To determine how much a dental procedure will cost and what the benefit amount will be, have your dentist submit a pre-determination/pre-estimate to receive an estimate of benefits available.
Yes. If you leave your job, you are able to take your coverage with you. If you leave your job, you will receive a Continuation of Coverage letter from Colonial Life with instructions on next steps. You will need to change your payment method to direct bill, which means you pay Colonial Life directly instead of paying through your employer.
