Medical BridgeSM 3000Medical BridgeSM 3000 features benefits for hospital confinement, wellness testing and rehabilitation unit confinement resulting from covered accidents and covered sicknesses. Additionally, Medical BridgeSM 3000 has plans that provide optional benefits in cases of covered accidents and covered sicknesses for:
  • Visits to a doctor’s office
  • Specified diagnostic testing procedures
  • Specified outpatient services

Coverage is available for you, your spouse and your dependent children. Plus, Medical BridgeSM 3000 offers several benefit amounts so that you can choose the coverage that best meets your individual needs. Medical BridgeSM 3000 is portable, which means you can take your coverage with you if you change jobs or retire.

Coverage is also guaranteed renewable for life as long as premiums are paid when they are due or within the grace period. Your premium can be changed only if we change it on all policies of this kind in force in the state where the policy was issued.

This coverage may not be available in all states; product benefits vary by state. Policies have exclusions and limitations that may affect benefits payable. For cost and complete details, please see your Colonial representative.

Medical BridgeSM 1000 Insurance

Medical BridgeSM 1000Medical BridgeSM 1000 features benefits for hospital confinement and outpatient surgery resulting from covered accidents and covered sicknesses, but with higher benefit amounts available for outpatient surgery. Additionally, Medical BridgeSM 1000 has plans that provide optional benefits in cases of covered accidents and covered sicknesses for:
  • Visits to a doctor’s office
  • Specified diagnostic testing procedures

An optional Emergency Care Rider is available with Medical BridgeSM 1000 plans at an additional cost. This rider pays benefits for visits to the emergency room, as well as transportation by ambulance or air ambulance in the event of a covered accident or a covered sickness.

Coverage is available for you, your spouse and your dependent children. Plus, Medical BridgeSM 1000 offers several benefit amounts so that you can choose the coverage that best meets your individual needs. Medical BridgeSM 1000 is portable, which means you can take your coverage with you if you change jobs or retire.

Medical BridgeSMBenefits from Medical BridgeSM are paid as a lump sum per covered hospital confinement or outpatient surgery and will be paid directly to you, unless you specify otherwise, regardless of any other insurance you may have with other insurance companies.

Medical BridgeSM provides supplemental benefits for:
  • Confinement to a hospital due to a covered accident or covered sickness.
  • Outpatient surgical procedures performed by a doctor, using anesthesia administered by a licensed anesthesiologist in a hospital or ambulatory surgical center.
The benefits from this plan can be used in any way you like including helping to pay the expenses related to a hospital stay such as:
  • travel expenses to and from the hospital.
  • lodging for your family members while you are in the hospital.
  • child care for your family.
  • other out-of-pocket hospital expenses not covered by your health insurance.

Coverage is available for you, your spouse and your dependent children. Plus, Medical BridgeSM offers several benefit amounts so that you can choose the coverage that best meets your individual needs. Medical BridgeSM is portable, which means you can take your coverage with you if you change jobs or retire.

Coverage is also guaranteed renewable for life as long as premiums are paid when they are due or within the grace period. Your premium can be changed only if we change it on all policies of this kind in force in the state where the policy was issued.

A choice of optional riders is available and can be purchased at an additional cost. These riders provide coverage for either accident or accident and sickness emergency room treatment and doctor office follow-up visits after a confinement or outpatient surgical procedure. A health screening rider is also available, to help you in preventive care for you or a covered family member.

Group Supplemental Health 1000 Insurance

Group Supplemental Health 1000Group Supplemental Health 1000 is a hospital confinement indemnity plan which employers can customize to meet the needs of their employees. The base plan offers benefits for hospital confinement, intensive care unit confinement, and inpatient or outpatient surgery with anesthesia resulting from covered accidents or covered sicknesses.

Depending on the major medical plan in an account, additional benefits may be offered to create a tailored package to help with the direct and indirect costs not covered by most major medical plans. These benefits include:

  • Doctor’s office visits
  • Preventive care tests including eye exam
  • Diagnostic tests, such as mammography, PSA, colonoscopy or CT scans
  • Prescription benefit
  • Accidental injuries, such as fractures and dislocations
Optional riders are also available.
  • Accidental death rider
  • Hospital admission rider (later in 2005)

Coverage is available for you, your spouse and your dependent children. Benefits are paid directly to the employee (unless otherwise specified) and are paid regardless of any other coverage the employee may have with other insurance companies.
