Short-Term Disability Insurance

Level Term Life Insurance

Level Term Life Insurance is affordable life insurance for the first-time buyer or someone who wants to supplement their existing coverage.

This Level Term Life Insurance plan provides the following features:
  • Higher levels of coverage for lower premiums, especially at a younger age when the need for protection is greater.
  • Level death benefits which means benefits will not decrease with time.
  • Premiums are also level, so they will not increase for up to two consecutive 10-year periods, depending on your age and the amount of coverage you choose. After the two consecutive 10-year periods, premiums will increase annually.
  • Family coverage available through non-convertible spouse or child term riders or an individual spouse policy.

  • Home or automobile modifications.
  • Mortgage, utility bills and other everyday living expenses.
  • Travel costs to and from treatment centers.

The benefits from this plan can be used where you need them most.

  • Pays a cancer screening benefit for specified screening tests.
  • Upon diagnosis of cancer, provides benefits for treatments and resulting costs that you may require to care for your cancer.

Our Specified Disease insurance helps you take the next step after diagnosis with comfort and reassurance. It pays a percentage of the policy face amount upon diagnosis of a covered specified disease, and you can use these
benefits to help pay the out-of-pocket expenses related to the treatment of your illness.

Benefits from our Specified Disease plan can help you pay the following types of out-of-pocket expenses:
  • Home health care or caregivers you may require
  • Home or automobile modifications

Hospital Indemnity Insurance

Going to the hospital is something you hope you or your family will never have to experience. This Short Term Disability Income Protection Insurance is designed to supplement your existing insurance coverage by replacing 30 to 50 percent of your income, based on income, if you become disabled. Benefits from this plan are paid on a monthly basis, up to $5,000 per month. Benefit periods range from three to 24 months.

Additional plan features:
  • You can choose the plan design that best meets your needs.
  • Worldwide coverage.
  • Sickness and on- or off-job accident disability coverage.
  • A choice of benefit elimination periods to best fit your needs.
Plan features:
  • Waiver of premium, which means that after you have been disabled for 90 days, or after the elimination period if longer, you do not have to pay your premium while you are receiving disability benefits.
  • Portable, so if you change jobs or retire, you can take this plan with you.
  • Premiums will not increase on existing coverage as you age, and your coverage is guaranteed renewable up to age 70 as long as premiums are paid when due.

Accidental death benefits can help your family meet continuing needs in case of an accidental death. The death benefit is increased if the accidental death occurs on a common carrier such as a commercial airplane or subway system.

Our accident plan features a flexible plan design so that you can choose the coverage that best meets your unique needs. Coverage is available for both on and off-job accidents, and several
optional riders are available so you can choose additional protection you or your family may need. The optional riders provide extra coverage and benefits for hospital confinement due to a covered sickness or disability due to a covered accident or a covered sickness. A health screening benefit rider is available to help with the cost of specified health screening tests.
This accident plan is:
  • Available for you, your spouse and your family.
  • Guaranteed renewable for life, as long as premiums are paid when they are due.
  • Portable. If you change jobs or retire, you can take your coverage with you.
  • Designed for your use. Your benefits will be paid directly to you unless you specify otherwise.
  • Premiums are level and do not change based on age or occupation.

    • This insurance is guaranteed renewable for life as long as premiums are paid when due or within the grace period.
    • This plan includes a waiver of premium. If you are disabled due to cancer for 90 continuous days after the effective date, you will not have to make the premium payments for this plan while you are disabled. The date of diagnosis of cancer must be after the waiting period and while the policy is in force.
    Additional Plan Benefits:
    This plan pays specific benefits for cancer diagnosis and treatment, including:
    • Wellness benefit: Payable for one of the specified cancer screening tests performed.

    • Bone marrow donor benefit: Payable if you donate your bone marrow to another person who has been identified as a match to your bone marrow type.
    • Inpatient benefits: Payable for hospital-related services such as hospital confinement, ambulance, air ambulance and full-time nursing services.
    • Treatment benefits: Payable for cancer treatments such as radiation and chemotherapy, antinausea medication and medical imaging.
    • Surgical procedures benefits: Payable for surgery performed to treat cancer, including reconstructive surgery and anesthesia.
    • Transportation and lodging benefits: Payable if you must travel to receive cancer treatment.
    • Extended care benefits: Payable for extended care services such as home health care, hospice and skilled nursing care.

    Our hospital indemnity plan can be used to help fill the gaps in your major medical plan and help pay the expenses related to a hospital stay.

    This plan provides coverage for the following:
    • Confinement to a hospital due to a covered accident or a covered sickness
    • Outpatient surgical procedures performed by a doctor using anesthesia administered by a licensed anesthesiologist in a hospital or ambulatory surgical center
    Benefits from our hospital indemnity plan
    are paid as a lump sum, per covered
    hospital confinement or covered outpatient surgery. These benefits can help pay copayments or deductibles, or help cover travel expenses to and from the hospital. These benefits will be paid directly to you, unless you specify otherwise, regardless of any other insurance you may have with other insurance companies.

    This plan features flexible plan options, and coverage is available for you, your spouse and your dependent children. Plus, our hospital indemnity plan offers several benefits amounts so that you can choose the coverage that best meets your individual needs.

    This hospital indemnity insurance is portable. If you change jobs or retire, you can take your coverage with you.
    Coverage is also guaranteed renewable for life as long as premiums are paid when they are due or within the grace period. Your premium can be changed only if we change it on all policies of this form number in force in the state of New York, subject to the prior approval of the Superintendent of Insurance.

    Additional riders are available with this plan. Coverage for emergency room treatment and doctor office follow-up visits after a covered confinement or covered outpatient surgical procedure can be purchased.
