Eligibility for Enrollment
  • Permanent full-time employees who work 20 or more hours per week are eligible for Universal Life, Cancer Response, Hospital Income Secure.
  • Eligible employees may apply for coverage on the 1st day of the month after 90 days of employment.

Permanent part-time employees who work 10 or more hours per week are eligible for ACCIDENT INSURANCE only.
