If you don’t find the information you are looking for here, you can call the Colonial Service Center at 1-800-325-4368. This center is dedicated to providing prompt, courteous and accurate information to


our policyholders. We set high service standards and work to exceed them.

When you call the service center you can choose the automated service option or choose to speak with a customer service representative. Please have your policy number or Social Security number available when you call.

For your convenience, the automated service option is available 24 hours a day, seven days a week. With our automated service option you can
  • Check the status of your claim or confirm the receipt of your claim.
  • Order additional claim forms.
  • Listen to instructions for filing a claim.
  • Listen to instructions on how to pay premiums while on a leave of absence.
  • File your wellness claim 24 hours a day, seven days a week.
  • Change your name or address.

If you require additional information and would like to speak with a customer service representative, please call the Colonial Service Center and a representative will be happy to assist you. Representatives are available Monday through Friday, 8 a.m. – 7 p.m. EST. Please have your Social Security

number or policy number ready when you call. To meet the needs of our Spanish-speaking policyholders, the Colonial Service Center is staffed with bilingual customer service representatives. These representatives are available during normal business hours and can be reached using the main number for the Colonial Service Center.
